Professionals Trained to Assist
Social Workers are professionals trained to assist individuals and families deal with the medical, emotional and environmental disruptions caused by a chronic, acute or terminal illness.
Our Medical Social Workers are a vital piece of the overall patient-care team at Gold Coast Home Health & Hospice. Besides providing psychosocial support to patients and families alike; Social Workers can identify and access available community resources; such as financial assistance, transportation, long-term care planning and placement, end-of-life planning, in-home care options, advance directives and living will information.
Home Health Aides
Home Health Aides (HHA), or Certified Nursing Assistants (CAN) provide in-home basic personal care to patients that have difficulty managing such activities and require more assistance than family and/or friends can provide.
HHA/CANs at Gold Coast Home Health work under the direction and supervision of professional staff in providing care such as bathing, skin care, shaving, ambulation, transfers from bed to chair, meal services, meal preparation and maintaining a healthy and safe environment.
